The Long Term Health Condition Grant is a funding opportunity to create programmes and activities that support adults with long term health conditions to keep active. Applications opened on 9 July 2026.
Applications close 5pm on Friday 25 September 2026 and applicants will be notified of the outcome on Monday 9 November 2026.
About the grant
Funding
- The maximum an applicant can apply for: £2000
- Usage: Venue hire, training courses, equipment purchases, accessibility requirements
- Scope: Single village or multiple villages
Grant objectives
- Supported Access to Activities: provide activities and programmes to support those with a wide range of long term health conditions stay active.
- Support Inclusivity: Encourage a wider range of activities across the district.
Grant benefits
- Promotion: Supported by us and partners via social media and other channels.
- Feedback: Providers must submit non-confidential participant information and session feedback.
- Health Questionnaires: New participants are encouraged to complete online health questionnaires.
Eligibility
The following people/groups or businesses can apply:
- Small fitness businesses/entrepreneurs (must show public liability insurance, qualifications, and safeguarding if applicable)
- Community facilities
- Voluntary sector organisations
- Specialist groups
Requirements
Presentation of relevant certificates before award and transfer of funds.
Sustainability
Applications should demonstrate a strong case for sustainability after initial funding.
