You can use this service to request a single change (variation) to a driver, vehicle or 
operator licence. If you need to make more than one change, please submit a separate 
request for each change you want to make. You must already hold a valid licence to use 
this service.

You cannot use this form to apply for a new licence. 

Changes you can apply for

Driver changes 

• change of name or address. You must notify us within 7 days of the change 

Operator changes

• change of operator trading name - you cannot transfer an operator licence
• change of licence holder name or address (this does not include business 
  address) - you must notify us within 7 days of the change
• change of operator for a driver or vehicle

Vehicle changes

• transfer a vehicle licence to another driver or proprietor - this must be submitted 
  no later than 14 days from the date the vehicle was sold
• change of vehicle registration 
• change of driver or operator(s) assigned to a vehicle 
• change from exempt to standard vehicle - you cannot carry out exempt work if 
  your vehicle is plated for standard work
• change from standard to exempt vehicle - you cannot carry out standard work if 
  your vehicle is licensed for exempt work
• change of name or address - you must notify us within 7 days of the change

• your licence number for the details you want to change
• any documents needed to support your change
• a payment method
