What is a Support Plan?
When you move into your home, the sheltered housing officer will contact you to prepare a support plan. Support plans are completed by residents of sheltered housing with the aid of your sheltered housing officer to help you to identify things you may need assistance with. They are confidential documents and the information is for you to stay independent and look after your health. Your sheltered housing officer can help in co-ordinating these services and linking with new ones.
We are required to record and monitor the services we provide to sheltered tenants to demonstrate that they are appropriate to individual's needs and wishes.
